This form is for builders who have paid their capacity and meter fees and are now ready for a meter to be set. Before completing this form, ensure the following to prevent delays in getting the meter set.

• Necessary capacity and meter fees have been paid.
• The meter box, setter, and lid are installed per Greenville Water specifications. Box must be level with finished grade and clear from debris.
• The address is clearly posted prior to the requested meter set date. Ensure all form fields are correctly entered below.
• I understand if the above conditions are not met, a $150 turn-down fee will be applied to the account.
